No Place Like Home offers a health insurance plan to all eligible employees. (minimum of 29 hours per week)
If you would like to enroll in the plan, employees must fill out both the health questionnaire and application. These documents, as well as details on the cost, coverage, and deductible information, are available online... Click HERE to view a SUMMARY of the plan benefits.
Enrollment is not mandatory; but each eligible employee must sign the waiver section each year declining coverage if you choose not to enroll in coverage. (located on pg 2 of the application)
